Case Management
Whether it’s a natural disaster, a health crisis, or feeling unsafe at home, our case managers support families and individuals to address the challenges they face. Please call us at (707) 965-5010 to speak with a Case Manager.
In-Home Appointment/Services
UpValley Family Centers’ Senior Service Case Managers offer in-home appointments for clients with mobility/transportation challenges in order to make services more accessible.
Assistance with In-Home Supportive Services
We provide general assistance to both providers and recipients regarding IHSS services. Our case managers can assist providers with completing the online application and making referrals to Napa County.
Urgent Needs Assistance
EMERGENCY ASSISTANCE PROGRAM
Our Emergency Assistance Program provides support for local older adults in times of crisis. Aid can come in the form of tangible goods, or in monetary aid. While our funding is limited, we also can help identify other resources that may be available.

Our Community Liaisons and other staff are available to provide you with guidance or assistance in completing applications for benefits or resources. Application assistance includes

ONSITE RESOURCES THROUGH PARTNER ORGANIZATIONS
We host onsite partners to provide services
- Community Health Initiative (CHI) appointments are available Upvalley during open enrollment season. CHI assists all Napa County residents to enroll in health insurance including Medi-Cal, Covered California, Private Individual and Family Plans and Medicare. For eligibility, they serve all ages and all incomes regardless of immigration status. There are no fees for their services.
- Medi-Cal and CalFresh application assistance is available at our Washington Street office (1705 Washington Street, Suite G, Calistoga, CA 9451). Any renewals for Medi-Cal may be completed and faxed from our St. Helena center (1440 Spring Street, St. Helena, CA 94574) to Health and Human Services Agency.
LINKAGE TO HEALTH SERVICES
UVFC staff can assist you with eligibility screening, and provide resource referrals to enroll in health insurance and/or to access covered care for physical, mental, and dental health needs.
Starting January 1, 2024, a new law in California will allow individuals between ages 26 and 49 to qualify for full-scope Medical regardless of their immigration status, and assets will no longer be counted. UVFC has staff members who are trained to complete new or renew Medi-Cal applications. We partner with Napa County Health and Human Services staff to address complex cases.
Social and Educational Activities
Social Activities
We host social activities and educational and health workshops specifically for older adults ages 55+ living in Calistoga as well as regular presentations and fun events at Calistoga’s mobile home parks – including art activities, brain fitness classes, and gardening projects.
Our Latina Women’s Group meets once a week for fun and activities. The women are encouraged to teach a class on their favorite hobby or interest. Guest speakers are selected to share information about mental health and other topics of interest.
Our Latino Men’s Group meets twice a month for an opportunity to socialize with other members of the community, learn about various resources, and dedicate time to their mental health and wellbeing.

Senior Focused Workshops
UVFC provides workshops for our older adults to help promote social connectivity education to resources and provide a safe space to gather and relate on common challenges around aging.
Our Lunch and Learn workshops in Calistoga are a collaboration between UpValley Family Centers, City of Calistoga Parks & Recreation, and Rianda House. Monthly meetings include an educational activity, a healthy lunch, and information about resources for older adults in Napa County.
Our Line Dancing workshops incorporate an hour of country line dancing, movement, and nutrition information for residents at Rancho De Calistoga and Chateau Mobile Home Park. For more information contact Devyn at dpoolos@upvalleyfamilycenters.org
